Parental/Guardian Consent: Medical, Participation and Travel
In the event of illness or accident, having parental responsibility, I give permission for medical treatment to be administered where considered necessary by a team mentor, or by suitably qualified medical practitioners. If I cannot be contacted and my child needs emergency hospital treatment, I authorise a qualified medical practitioner to provide emergency treatment or medication. I consent to the above child(ren) participating in (and travelling to) the activities of Kevins Hurling & Camogie Club in line with the GAA Code of Best Practice in Youth Sport. Kevins is pleased to confirm the implementation of Garda Vetting in the Club as we promote best practice in the recruitment and selection of persons to work with our members.
