I agree to one of the Club’s coaches to sign on my behalf any written consent form which may be required by any hospital/doctor, in the event of illness or accident to the Player that requires emergency hospital treatment, should the delay required in obtaining my signature is considered inadvisable by the doctor or surgeon concerned.
