Claregalway GAA Agreement

I hereby apply to Claregalway GAA (“the Club”) for Membership of the Club and Membership of the relevant GAA governing body.

I subscribe to and undertake to further the aims and objectives of the Club and of the relevant GAA governing body and to abide by its Rules.

I will pay the appropriate membership fee as determined by Claregalway GAA.

• I understand the personal data on this form (“Personal Data”) will be used by the Club and the relevant GAA governing body for the contractual purpose of registering (or re-registering) and maintaining my Membership.
• I understand that the Personal Data will be retained by the Club and the relevant GAA governing body for such period as my Membership subsists.
• I understand that I can resign my Membership by writing to the Club or the relevant GAA governing body and my Personal Data will then be erased.
• I understand that my Personal Data will also be used for administrative purposes to maintain my Membership including registrations, team-sheets, referee reports, disciplinary matters, Injury Reports, transfers, sanctions, permits and for statistical purposes.
• I understand that if I do not provide my Personal Data my Membership cannot be registered with the Club and the relevant GAA governing body.

I give my consent, for my information to be used as follows:

1. To provide me with updates regarding Club activities such as matches, meetings and club events
2. To provide me with details of Club fundraising activities including Lotto, social occasions, ticket sales.
3. I am aware that my photograph or video image may be taken whilst attending or participating in games or activities connected with the relevant GAA Governing body and I consent to it being used by the relevant GAA governing body for items like match programmes, year books, match reports, event reports or on websites or social media channels associated with the relevant GAA governing body
4. To contact me in relation to volunteering opportunities at relevant GAA governing body events and activities

I understand that I can withdraw my consent at any time by writing to the Club or the relevant GAA Governing body and in the case of communication preferences by using my online Clubforce Account.

I understand my rights under Data Protection legislation, which are outlined by the GAA HERE and the LGFA HERE

In addition and in the case of a Juvenile registration:

I give the club permission for my child to be brought to Doctors / Hospital in the event I cannot be contacted if medical attention is needed.

The club would ask that you advise the relevant coach if your child has a medical condition that they would need to be aware of.

I give permission for the club to take and publish photos of myself or my children. Some of these pictures are published in the local newspapers, and put on the Club Website/Social Media channels. The club will act on requests to remove specific photos if asked.
