Medical Conditions/ Permission, and Health & Safety
I give permission for my child to brought to a doctor or hospital in case of emergency and authorise all medical / surgical procedures as advised by the treating doctor.
I understand that it is the Parent/Legal Guardian’s responsibility to inform the team Coach/Trainer or Volunteer of any medical conditions or special needs this child might have. Please choose “Agee” below, if you agree to do so.
I also acknowledge that Three Rock Rovers Hockey Club’s policy is that shin guards and gum shields must be used by players at all times and it is not the club’s responsibility to ensure that these are being used by the player.
