Lucan AC Terms & Conditions

Membership will be bound by the rules of the club and membership is granted on the understanding that applicants join at their own risk and neither Lucan Harrier AC, their Coaches, Agents or Servants will be held responsible or liable for any accident or injury to any member or the property of members, whether damaged, lost, mislaid or stolen, even if caused by negligence or breach of statutory duty on the part of the above mentioned.

Parent/Guardian:

• In the event of illness or accident, having parental responsibility, I give permission for medical treatment to be administered where considered necessary by a nominated first aider, or by suitably qualified medical practitioners.  If I cannot be contacted I give permission for club coaches and club officials to act on my behalf to seek medical assistance. If I cannot be contacted and my child needs emergency hospital treatment, I authorize a qualified medical practitioner to provide emergency treatment or medication.
• I have read and understand the code of Ethics and Good Practice for Juveniles and agree to follow said code at all times.
• I understand that photographs or film may be taken during or at sport related events and may be used in the reporting or promotion of the sport.
  (If you do not wish your child to be photographed or filmed please advise the club Children’s Officer)

Medical conditions and allergies:

• I declare all medical conditions and allergies relevant to taking part in athletics, and promise to inform the club and coaches of any changes to such conditions and allergies.
